2008 Buddy Form - Optional form for all Mail in and Onsite Registrations for T-Ball and U8 Softball players who wish to play with a friend (Forms will be availible at the Onsite Registration)
2008 Coaching Application - All People wishing to be a league coach must complete this form
2008 Umpire Application - All people 12 years old or older wishing to umpire at Kennedy Little League must complete this form

League Communication Form - This form should be used to communicate Suggestions, Concerns or Compliments (Forms not signed will be discarded)

Medical Release Form - Every Player will need a Medical Release form completed and coaches will need to bring them to every game. This is part of our safety plan and a requirement Little League international.
